1. What personally identifiable information of yours is collected from you by OKTV, Inc. or a third party?

Your child's age and a grade, in order to make program recommendations that are most likely to be appropriate for your child.
A user-name for your child, in order to personalize your child's program guide. You can use any nickname or other name instead of your child's name.
Your name, for billing purposes.
Your zip code, in order to know what cable or satellite provider services your area so that we can select the correct program listings for your area.
Your email address, in order to communicate with you as service or other issues arise.

2. How is the information used?

Registration
When you first register for OKTV® we will ask you for some personal contact information. This information is used solely to provide the services for which you subscribe. We need this information either to provide the personalization service or to contact you about issues as they arise. Demographic information (such as number of children in your family) is optional but encouraged so we can provide a more personalized experience on our site.

Surveys & Contests
From time-to-time OKTV, Inc. requests information from users via surveys or contests. Information requested may include contact information (such as name and shipping address), and demographic information (such as zip code, age level). Contact information will be used to notify the winners and award prizes. Survey information will be used for purposes of monitoring or improving the use and satisfaction of this site. Participation in these surveys or contests is completely voluntary and the user therefore has a choice whether or not to disclose this information.

Special Offers
We send all new members a welcoming email to verify password and username. Established members will occasionally receive information on products, services, special deals, and a newsletter. Site and Service Updates
We also send the user site and service announcement updates. Members are not able to un-subscribe from service announcements, which contain important information about the service. Where there is advertising on OKTV® pages, the advertiser and/or the advertising broker may collect data about the users viewing those pages. While OKTV, Inc. cannot assume responsibility for the policies of advertisers or their brokers, we will do everything we can to prevent advertisers and their brokers from matching the anonymous data they may collect on through OKTV® with personally identifiable information they gather elsewhere. Cookies
A cookie is a piece of data stored on your hard drive containing information about your use of the site. Information contained in cookies is in no way linked to any personally identifiable information while on our site. OKTV, Inc. uses cookies to personalize the service for a particular user. If a user rejects the cookie, they may still use our site. The only drawback to this is that the user will have to re-register each time he or she returns to the site. In order for this website to properly fulfill its obligation to our customers, it is necessary for us to share the information we receive with 3rd party sources. For example, in order for your cable provider to bill you for your subscription to OKTV®, we must notify the cable provider that you are OKTV® subscriber.

OKTV, Inc. distinguishes personally identifiable information from information about anonymous groups of people--aggregate data. OKTV, Inc. will never provide third parties data that will in any way personally identify parents, children or the household without the expressed consent of at least one parent or legal guardian. This includes, but is not limited to, email addresses, home telephone numbers, street addresses and names.

6. What choices are available to you regarding collection, use and distribution of the information

OKTV, Inc. gives users the opportunity to 'opt-in' to having their information used for purposes not directly related to our site at the point where we ask for the information. For example, our order form has an 'opt-in' mechanism so users who register with us will only receive our news letter if they choose to do by 'opting in.'

Users who have opted in and no longer wish to receive OKTV, Inc.'s newsletter or promotional materials from our partners may opt-out of receiving these communications by replying to unsubscribe in the subject line in the email or email us at support@OKTV, Inc..com

OKTV, Inc. always notifies users when their personally identifiable information is being collected by any outside parties. We do this so our users can make an informed choice as to whether they should proceed with services that require an outside party, or not. Top

OKTV, Inc. takes every precaution to protect our users' information. When users submit sensitive or personally identifiable information to OKTV, Inc., that information is protected both online and off-line.

When our registration/order form asks users to enter sensitive information (such as credit card number and/or social security number), that information is encrypted and is protected with the best encryption software in the industry - SSL. While on a secure page, such as our order form, the lock icon on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked, as opposed to un-locked, or open, when you are just 'surfing'.

While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user-information off-line. All of our users' information, not just the sensitive information mentioned above, is restricted in our offices. Only employees who need the information to perform a specific job (for example, our billing clerk or a customer service representative) are granted access to personally identifiable information. Our employees must use password-protected screen-savers when they leave their desk. When they return, they must re-enter their password to re-gain access to your information. Furthermore, ALL employees are kept up-to-date on our security and privacy practices.
